Making the Most of Your Patient Journey

When living with a long-term health condition, ongoing medical care is important for achieving the best results for your health. PRO Patient Companion is a convenient and effective toolkit designed to help you manage your health by tracking what medications you are taking, what symptoms you may be experiencing, your quality of life, and your appointments with your clinician/healthcare team.

Importantly, you will be able to share your health information (data) in a secure (HIPAA-compliant) manner that ensures your privacy and clearly communicates a more accurate picture of your “health story” to your care team. This enables your team to identify areas of concern and manage those more effectively leading to an improved health journey.

NOTE: This app integrates with the Health app so if you choose to manage certain heart conditions and have an Apple Watch or other device that creates heart rate data, you can send that data to your doctor through the app giving you full control over which data you send. No health data is collected or sent from device without you specifically doing it.

• Report your quality of life by completing surveys and questionnaires
• Share meaningful, easy-to-read charts with your care team, so they can monitor your progress between visits and better understand how your condition is affecting you
• Stay up to date with the latest information on your condition(s) when you access the “Learn More” section
• In-app Help guides you through the features so you can get the most out of the app
